Ability to pay is not a consideration for admission. Fees for services will be charged depending on financial ability to pay according to the Fee Determination form (UMDAP) and Fee Agreement form. If applicable, participants may schedule a weekly, monthly, bimonthly or in full payment schedule. Medical eligibility constitutes form of payment.
